β’ Rubella (German measles) is a mild viral illness caused by the rubella virus.
β’ Usually self-limiting in children, but severe fetal complications can occur in pregnancy (Congenital Rubella Syndrome β CRS).
π Causes & Risk Factors
β’ Rubella virus (respiratory droplet spread)
β’ Biggest risk: Unvaccinated pregnant women
β’ Other risks: Immunosuppression, travel to areas with low vaccine uptake

π Pathophysiology
β’ Virus infects respiratory epithelium β lymph nodes β bloodstream
β’ Crosses placenta β damages fetal organs β CRS
π Symptoms
β’ Mild fever, conjunctivitis, malaise
β’ Pink maculopapular rash: starts behind ears/face β spreads down
β’ Lymphadenopathy: cervical, suboccipital, postauricular nodes
β’ Arthralgia common in adults
β’ Forchheimer spots (palate) may be present
π§ Visual tip: Rash flows down the face like a waterfall π
π Differential Diagnosis
β’ Measles, roseola, parvovirus B19
β’ Enteroviruses, adenovirus, CMV, HIV
β’ Dengue, Zika, chikungunya (travel history!)
β’ Scarlet fever, Kawasaki disease, syphilis
β’ Drug reactions (e.g. from antiepileptics)
π Diagnosis
β’ π¬ Serology β IgM rubella antibodies
β’ π§ͺ PCR β viral RNA detection
β’ FBC: lymphocytosis, thrombocytopenia
β’ CRS: antenatal ultrasound, virology referral
π§ Tip: Always notify suspected rubella (UK notifiable disease)
π Management
β’ π‘οΈ Supportive only β rest, fluids, antipyretics
β’ β Avoid aspirin in kids (Reye's syndrome risk)
β’ π« Exclude from school 5 days post-rash onset
β’ π€° Urgent referral if suspected in pregnancy β involve ID & obstetrics
π Complications
β’ CRS: congenital deafness, cataracts, CHD, intellectual disability
β’ Others (rare): encephalitis, arthritis, thrombocytopenia, GBS
π Prognosis
β’ Children/adults: Excellent β usually self-resolving
β’ Fetus (if infected): Poor β especially in first trimester
