π§ What Is Trigger Finger?
Inflammation causes thickening of the tendon or sheath, leading to a mismatch in size at the A1 pulley.
π’ This causes a snapping or locking sensation when the finger moves β it might get stuck and then suddenly pop free.
π Causes & Risk Factors
β’ Repetitive hand movements (e.g. gripping tools)
β’ Rheumatoid arthritis
β’ Diabetes mellitus
β’ Age >40
β’ Female sex
β’ History of hand trauma
β’ Anatomical variations
βοΈ Pathophysiology
Think of the tendon as a rope, sliding through a tunnel.
πͺ€ A nodule (speed bump) on the tendon catches at the pulley entrance β snap, click, or lock.
π©Ί Symptoms
β’ Pain/tenderness at base of the finger or thumb
β’ Stiffness, worse in the morning
β’ Audible/palpable clicking
β’ Finger may lock in flexion
β’ Often affects thumb, middle, or ring finger
β’ May feel a palpable nodule over the A1 pulley
π Differential Diagnosis
π§ Donβt confuse it with:
β’ Osteoarthritis
β’ Dupuytrenβs contracture
β’ Flexor tendinitis
β’ Ganglion cyst near tendon
β’ Nerve compression

Affects ~2β3% of the UK population

More common in women >40

Associated with systemic inflammatory diseases (RA, diabetes)
π§ͺ Diagnosis
β’ Clinical diagnosis based on history + exam
β’ No imaging usually required
β’ Ultrasound/MRI only if diagnosis uncertain
π Management Options
1οΈβ£ Conservative First-Line
β’ Rest
β’ Ice
β’ Splinting (especially at night)
β’ NSAIDs
β’ Hand therapy
2οΈβ£ Corticosteroid Injection
β’ Injected into the flexor sheath at the A1 pulley
β’ Often very effective β relief within days to weeks
β’ Can be repeated if needed
3οΈβ£ Surgical Release
β’ Minor procedure under local anaesthetic
β’ Cuts the tight pulley to allow smooth tendon glide
β’ Reserved for refractory or severe cases

π Good with early treatment
π Recurrence possible if underlying causes persist
