Beyond the Bedside Chapter 19: Evaluating (Close the Loop on Care)

Evaluation is where nursing proves it works. In this episode, we master the fifth step of the nursing process—writing tight evaluative statements, choosing concurrent vs. retrospective evaluation, and knowing exactly when to terminate, continue, or modify a plan. We walk through three full cases (Tyler the neonate, Mioshi on diuretics, Nicholas with new diabetes) to show how real bedside data drives smarter decisions. Then we scale up with QI/PDSA, NDNQI indicators, HCAHPS, IOM’s 10 Rules, the AACN Healthy Work Environment standards, and the Seven Crucial Conversations so speaking up becomes standard practice—not a gamble.

You’ll learn:
• How to write bullet-proof evaluative statements (date + verdict + evidence)
• When to use concurrent vs. retrospective evaluation
• Fast plan revisions using Table 19-2 logic
• QI that actually sticks (small tests, measured change)
• Safer teamwork using Silence Kills + AACN HWE

NCLEX Boost: sequencing the 5 evaluation elements, domains of outcomes, and “best next step” logic when outcomes are only partially met.
