The answer is Two! ‘どちら さま ですか？’ is natural Japanese.

If you do not know what kind of relationship the person you are meeting for the first time is to you, you need to phrase it in a way that is not rude, just in case.
The three phrases are about as different in English as ‘Who are you?’ and ‘May I ask your name?’
Let's be sure to speak politely.
初対面の人が、自分にとってどんな関係の人間かわからない場合は、念のため、失礼でないフレーズが必要です。
３つのフレーズは、英語で言うと、“Who are you?”と”May I ask your name?”くらい差があります。
丁寧に話すように心がけましょう。
