There is but one underlying reason why 92% of people who set goals never achieve them and that is because of belief. Rather if it is beliefs we hold about ourselves others, or the world around us, beliefs shape our perspectives. Why is this important? Well, untimely it is perceptions/perspectives that shape our reality, the reality that dictates our actions, and our actions become our life. Thus, if you want to change your life aka the results you are getting, you HAVE to change your beliefs.
