Well you go into your search terms report and you will find which search terms triggered your ads and you can also go in the keyword tool or Keyword Planner and find your search terms very few people do this that's the thing is you got to go into your search terms report regularly.
If it's a small account I would recommend at least once a week if it's a bigger one every day if not every other day. And keep finding search terms which are not good. Put them in negative so you are stopping the leakages and the wastage in your account and over a period of time, you're going to have thousands and tens of thousands of negative keywords which will make your account super optimized.
