Part 2: RPM Panel Experts
* Sid Kandan, Co-Founder & CEO at Stel Life, Inc.
* Sid Kandan founded Stel Life to simplify connected health for patients, care teams, and manufacturers. Having learned to program because of various Bluetooth projects from his time at Duke, Singularity University, and Facebook. He saw the potential for IoT to improve the standard of care. Together with health system leaders and talented engineers, they created Stel to build technology to establish the infrastructure needed to evolve care to the home. With Stel’s patented technology - patients wouldn’t need Wi-Fi, smartphones, or tech literacy, Care-teams could use their EHR or preferred workflows, and Health orgs wouldn’t risk PHI or supply-chain flexibility.
Sid is an active contributor to discussions about the digital divide, IoT, wireless devices, and new programs to advance technology to improve health equity and access to care for vulnerable populations. As a serial tinkerer, Sid occasionally finds time to tinker on creations like his: Ye.i. Home Automation Robots, talking dog collar, or his own fan-cut rendition of the Hamilton musical. * Stel Life: Simplifies connectivity for patients, care teams, digital health, and device companies. Healthcare leaders are struggling with the reality of the digital divide and integrating hundreds of point solutions across the care continuum. With Stel, patients don’t require Wi-Fi, smartphones, or tech literacy to connect with their care teams. Patients use health devices near a Stel Node. Stel passively and securely connects the device data to their care team’s Electronic Health Records (EHR) or preferred platform without risking Protected Health Information (PHI) or complicated workflows.

* Robin Henry, RN, Chief Population Health Officer with Solvera Health
* Robin has been a Registered Nurse for 20 years, primarily serving the aging population in Illinois. The niche she built for her scope of work has been strategic and impactful, developing strong relationships in the healthcare space from years as a Director of Nursing to her ongoing provider-facing work teaching acute care providers how to navigate the hospital discharge process to the post-acute landscape, pre, during and post-COVID19. She brings with her resource allocation, analytic driver, and innovative disruption, "deploying RPM before it was cool," opening the digital front door, piggybacking a person's transition of care as an operational manager of Acute Digital Care, identifying means to serve patients out of the traditional brick and mortar. Her most recent role is that of Chief Population Health Officer with Solvera Health, bringing value and experience, elevating the focus around Chronic Care Management, Value-Based Care deliverables, and community outreach. Robin is a fierce advocate for access to care and ease of access, and digital care is a key driver for so many, the "culture of convenience," for example. Serving many of Illinois's healthcare initiatives from regulatory and reimbursement roadblocks around Serious Illness/Illinois Hospice and Palliative Care Organization, Nursing Home Reform, and Medicaid reform.
* Solvera Health- Solvera Health has a unique entrance into healthcare at the cusp of the PHE. The owners have had their footprints in the restaurant industry for over 25 years in central Illinois. The space we occupy in Peoria was a previous restaurant of the owners that is now a comprehensive, "all under one roof" twelve thousand square feet of clinic: including lab, diagnostic, pharmacy, and specialty service lines Urology, Gynecology, and Audiology. The pandemic revealed the community's need for access to care. The building was put to use vs. vacant. Providers, clinic, and administrative staff embraced their new roles. Changing the windshield to "Hospitality in Healthcare" Our first guest encounter was 10.24.2021, and the team stood up two more primary care clinics in our first year. (Galesburg and Carbondale, Illinois). We continue to elevate what hospitality means in healthcare as we serve our communities—bringing innovative, out-of-the-box thinking and finding our way to yes.

* Welby- Welby Health is a tech-enabled service provider offering case management services to physicians. Its products generate revenue for its clients by leveraging reimbursement for Remote Monitoring, Chronic Care Management, and virtual care codes made available through its tech platform. We use proprietary technology and a team of nurses trained in remote patient engagement to leverage billable codes on FFS business and improve clinical outcomes to maximize VBC bonus programs. The WelbyCare program is a turnkey solution for providers who need additional support to achieve improved HEDIS metrics but do not have the necessary capital to implement a new program from scratch. We understand the busy workflows of primary care providers, so we have designed our system to allow physicians to implement the program in under 30 days without any upfront investment and no changes to your existing workflow. We can handle everything from patient identification and onboarding through billing while our clients can focus on doing what they do best, delivering hands-on care to their patients.
