Before and after his dysphoria was cured.
Now he's got iatrogenic sanpaku eyes. To me this creates the illusion that he's actually wearing his mother's flayed face as a mask.
Seems that he's got some muscle weakness and droop affecting the zygomaticus muscle and zygomatic branch of the facial nerve.
This is consistent with his report if a life-threatening oral arterial bleed during the surgery. I actually noticed this under the Halloween mask. It just looked droopy.
