A near-miss on the South-Eastern Freeway this week shows how 2015 Coroner recommendations are still being ignored, according to the TWU.

On Wednesday morning a truck driver slammed into a pile of gravel near the Heysen Tunnels as he believed he was experiencing brake failure.
