[00:01] Welcome to another episode of the recharge podcast. This is segment six of behind the bedroom door becoming the bedroom hero. What I want to talk about in this segment is a technology that's actually been around for quite a while. It's been used in medicine and healthcare for a long, long time for a variety of applications, but it's only recently gained light, probably partly due to marketing efforts and promotional efforts, but also due to need and what I'm talking about is sound wave therapy. There are several companies out there that manufacture these devices. If you're familiar with lithotripsy, it's actually a technique where sound waves are directed into the kidneys to break up kidney stones. It's done under general anesthesia because of the pain, but it's in a very effective way to help rid the body of stones in the realm of behind the bedroom door. [00:49] It deals with erectile dysfunction and the rationale behind it, as it was explained to me by several physicians who are experts in this realm, is that it breaks up scar tissue, stimulates new blood vessel growth in, can improve blood flow, and so the treatments are painless and take about 20 minutes and then directed not only to the penis itself, but also at the base of the penis and just right in that region with the realization that blood flow, obviously he comes from higher up and depending on the mechanism, whether it's pelvic trauma, surgery, radiation, or something else. This device has shown promise in improving blood flow, which is the key heart of the matter in terms of erectile dysfunction. So there's a variety of other adjunctive techniques that I'll share here in just a moment that are used in combination with this device in that, and those include prp and stem cells. [01:42] So prp stands for platelet rich plasma and this has got its start, I believe in. You know, mostly in the joint space. There was an older technique called prolotherapy, but the reason is that platelets, the little sticky things in our blood are just packed with growth factors and by injecting these in a variety of tissues, whether it's joined the penis called the p shot or on the female, the Oshot can actually cause some improved physiologic responses, and so what what skilled providers are doing now is they're combining the shockwave therapy with either prp injections or stem cell injections and generating in demonstrating some pretty impressive results for people with a, with a relatively refractory erectile dysfunction, so it's a way to improve what the body does naturally without necessarily relying on a drug such as a Sildenafil, Tadalafil, et Cetera, et Cetera. There are a variety of other similar acting drugs out there, so some people want something that's more lasting or long term and this has the potential application for that. [02:43] Not every patient is right for this therapy. You're going to want to see somebody who knows what they're doing and has experienced in this realm. The final point I wanted to bring up and bring out is that a prp. Well, two things actually, prp and stem cells are different. PRP is platelets. Stem cells are actually derive from your fat and it's a different technique of extraction and reinsertion, so keep that in mind. They're completely separate and different, but they both can be used in the realm of erectile dysfunction. PRP is a math problem. You need a provider that really knows what they're doing in terms of the math and what I'm getting at is that there is a minimal quantity of platelets that are really necessary to generate an improved result in, so just taking a little bit of platelets and injecting them into the tissue, whether it's the base of the penis, I'm a joint, et cetera. [03:36] If it's not a high enough concentration and the result is going to be less robust or even nonexistent, so it's not a cheap procedure, and so I really encourage you to question the provider that you're seeing about the technique, the technology, that protocol, the results they've gotten in the past to make sure that you are getting the best use of your money. So that wraps up this segment. In the next section, I'm going to be talking about some common mistakes. I alluded to this a little bit in the earlier section, but I want to just to expand on this a little bit here and then we'll be in the home stretch where I will, uh, bring things together and start wrapping things up for you.
