The Ponzi scheme fell apart with the financial crisis and stock market crash of 2008. When Madoff ran out of options and knew that the jig was up he confessed to his crimes and plead guilty. The documentary covers Madoff's entire life and details how the Ponzi scheme operated and how it was able to keep going for decades.
