When push comes to shove
means when a situation becomes serious or difficult and a real decision or action is needed. It’s what happens when you’re under pressure and have to act.
Examples:
1- Many people complain about the system, but when push comes to shove, very few take real action to change it.
2- They promised to help, but when push came to shove, only one friend showed up at the hospital.
