Description

What does it look like to reshape the experience of scheduling appointments in a way that enhances the continuity of care? Patients manage competing commitments and may face certain barriers to make appointments and follow-ups. Community health center staff members are tasked daily with a large volume to meet the often insurmountable expectations of appropriately matching a given patient to the correct provider to a timely appointment. 

Petaluma Health Center took on this challenge leading with a data-driven and patient-attuned mindset aimed to optimize appointment utilization while also better enriching staff capability despite limited human resources, and overall ensuring for greater access to health.
