The issue stems from insufficient training at a Tier 1 supplier, Otsuka Koki Co., where new employees failed to properly stake brake pedal pivot pins during a production transition. This manufacturing error was compounded by a disabled QR code scanning system, preventing detection of the faulty parts. While three warranty claims related to brake pedal movement have been reported, Honda is not aware of any injuries, and dealers will replace affected brake pedal assemblies free of charge.
