Description

If you feel like you can’t eat much and the scale still will not move, you may not be failing, you may be dealing with a slower-than-expected metabolism. I’m Dr. Lindsay Ogle, a board-certified Family Medicine and Obesity Medicine physician, and I’m walking you through the fourth obesity phenotype: slow burn. It is one of the most frustrating patterns I see because it can make weight loss and weight maintenance feel brutally unfair.

We unpack what “slow burn” really means in plain language: a lower resting metabolic rate, or fewer calories burned at rest. Then we map out the biggest reasons it happens, including genetics, aging, decreased muscle mass, and a history of severe caloric restriction (yes, the Biggest Loser-style approach can leave a lasting metabolic imprint). This also explains why some people do not respond as expected to GLP-1 medications like Wegovy, Zepbound, Ozempic, or Mounjaro when slow burn is a major driver.

From there, we get practical. Since we do not currently have a specific medication that “treats” slow burn directly, we focus on the interventions that work: resistance training you can actually stick with, adequate protein to support muscle-building (especially during fat loss and on GLP-1 therapy), and NEAT, non-exercise activity thermogenesis, to increase daily calorie burn without living in the gym. I also share a few programs I recommend to patients for guided, at-home strength training.

If this helps, share it with someone who’s stuck in a weight loss plateau, then subscribe and leave a review so more people can find evidence-based metabolic health guidance.
