Y博的科普园（新冠资讯分享）强生疫苗GBS风险、免疫缺陷人群的增强针与以色列疫苗39%有效性（07/24新冠资讯分享录音）7月22日的CDC的ACIP会议有两大主题：强生的GBS与免疫缺陷人群是否需要增强针，注意是免疫缺陷人群，不是普通人群。提供了很多有用的信息。1.强生的GBS问题Guillain-Barré 综合症：一种罕见的神经系统疾病，致病原理是免疫系统攻击神经，导致肌肉无力，严重时可能导致瘫痪，从原理上讲是一种自身免疫疾病。具体导致疾病的原因并不清楚，在美国一年会发生3000-6000例，主要由呼吸道或肠道感染诱发。绝大部分人可以恢复，但少部分人会遗留终生的神经损伤。在50岁以上以及男性中风险更高。截至6月30日，VAERS系统中共报告100例强生疫苗接种后发生的GBS，其中95例属于严重性质，需要住院，1例死亡。需要注意这是接种后发生的粗略统计，还没有完成所有的病例核实。平均年龄57岁，100例中61例为男性。84例发生在接种后21天内，98例在接种后42天内。95例住院的严重病例里10例需要插管，死亡的一例为57岁男性，有多种基础疾病包括心脏衰竭、中风史。100例GBS对应的1200万剂的强生疫苗接种总数。GBS的背景发生率根据年龄段大概在每十万人中1-2例的范围。强生疫苗的数据对应下来，各年龄段都超出了背景值，40-64岁在背景值的6-7倍左右。相比之下辉瑞与Moderna分别在VEARS中有190与162例GBS，不过接种率远高于强生，发生率与背景值接近。主动跟踪的系统VSD里，强生的GBS发生率也高于mRNA疫苗，但由于总体强生接种量太少，无法确认风险，需要继续跟踪同时，截至6月27日，EMA收到227例阿斯利康疫苗接种后的GBS病例，对应5100多万剂接种。现在FDA与EMA都把GBS列入了强生与阿斯利康疫苗的风险中。2.强生疫苗收益仍远大于风险强生针对有症状新冠的保护在临床试验里是66%，对住院保护是93%。要注意这些是在已经有多个突变株出现的时候做出来的研究结果，对现在仍然是有指导意义的。现在接种超过1200万剂强生疫苗，两个严重不良反应，TTS报告了38例， GBS是98例。TTS风险最高30-39岁女性，每百万是8.8例，GBS男性50-64岁最高，每百万15.6例。对于30-39岁女性，每接种100万剂强生疫苗，会有6-7例GBS，8-10例TTS，但减少了一万多例新冠，900例住院，140个ICU病例，20例死亡。对于50-64岁男性，每接种100万剂强生疫苗，有14-17例GBS，1-2例TTS，但减少一万多例新冠，1800例住院，480例ICU，140例死亡。对于mRNA疫苗，目前的严重不良反应是心肌炎，根据截至6月30日的统计，这个最高的风险在18-29岁男性，每百万剂接种对应24.3例，但可以减少9600例感染，300例住院，60例ICU，3例死亡。需要注意这些收益还是低估的，比如没有考虑减少了新冠后遗症。所以即便是有GBS与TTS，强生疫苗的收益远大于风险。mRNA疫苗的心肌炎也是如此。所以CDC强烈支持继续使用强生疫苗，并鼓励所有人接种新冠疫苗。3.免疫缺陷人群的增强针问题2.7%的美国成年人属于免疫缺陷人群，包括肿瘤患者、接受了器官移植的人、先天免疫缺陷、HIV患者、使用免疫抑制药物的人。特别值得注意的一个数据是免疫缺陷的人现在突破性感染很多，美国44%的突破性感染住院病例是这个人群，以色列是40%。有多项研究也显示同样接种两针mRNA疫苗，免疫缺陷人群的疫苗有效性要低于一般人群。一些研究也显示，如果接种第三针mRNA疫苗，之前没产生抗体的严重免疫缺陷病人中33%-50%可以产生抗体。以上的证据说明免疫缺陷人群确实风险更大，即便打了疫苗也是如此，然后增强针可能可以有一定帮助。但要到具体执行增强针还有多个问题需要解决。首先，免疫缺陷人群里的具体哪些需要特别关注，器官移植的应该肯定属于，但使用免疫抑制药物的个体差异非常大，这时可能要非常依赖直接医疗团队的判断。其次，抗体滴度与保护作用的关系还未完全建立，特别是对于免疫缺陷人群，像器官移植的试验里，第三针后抗体滴度也不高，这种情况下，很难根据增强针诱发抗体反应来确认是否就是有足够的保护力。而且一旦推广，还有现存商用抗体测试结果不一致，并不适合用于确认免疫反应的问题。最后，FDA现在批准的分别是两剂与一剂，要改的话需要通过改EUA或正式BLA获批，后者就可以通过off label机制增加一针。ACIP会议指出需要继续收集资料，研究增强针混打问题，以及确认哪些免疫抑制人群需要第三针。但允许第三针要等BLA。同时，ACIP也再度重申了免疫缺陷人群要继续做好个人防护，同时家庭成员要接种疫苗。这些建议很合理，最后的决策——也就是继续等，也不算有问题，但决策时把主要原因推到需要等BLA这个我个人认为太保守，也不是最合理的做法。BLA现在最快的是辉瑞，也是基于普通人群的三期临床数据，最后也是两针的。免疫缺陷人群的问题与BLA，审批上确实有关联，过了BLA，就是完全上市的药物，打第三针可以算到off label里。但现在免疫缺陷的人是保护不足，需要从科学上找到合适的帮助手段，这个时候说我们先等BLA，等于是在踢皮球，把问题踢给FDA了。其实会议里也提到了单克隆抗体是不是用于被动免疫，来起作用。Regeneron在4月的时候就申请作为预防使用。这方面其实可以更激进一些。4.面对疫情反复，最好的做法仍然是提高疫苗接种率7月17日时，Delta已经占了全美新增病例的80%。每日新增现在又回到了6万的水平，而这里面很多来自几个接种率最低的州。截至7月21日，美国18岁以上成年人中68.4%至少接种过一针，完全接种完的是59.6%。接种速率的增长仍然非常缓慢，这是非常令人遗憾的，也是非常危险的。最近一些研究的结论下得有点大，引起了一些不必要的焦虑。一个是强生疫苗对Delta的保护作用，纽约的一个研究组，用接种完80-90天的血清，发现针对Delta的中和能力相对原始病毒下降了5倍，根据Nature medicine那篇论文的建模，部分人下降到了不能防护有症状感染的地步。研究人员提出强生疫苗对Delta有效性低，需要增强针。这个研究被纽约时报报道，也引发了不少关注。首先，这和之前强生临床试验志愿者的血清中和实验是有冲突的，为什么做出来会不一样，需要更仔细评估，不应该仓促以一个实验下结论。其次，更为重要的是，根据三期临床试验里观察到的真实有效性，强生对于免疫逃逸普遍认为最严重的beta也是有效的，而且也是一针。现在不少体外血清中和实验拿中和抗体随时间下降作为有效性下降的依据，这种研究是非常片面的。不应该用来取代实际的有效性观察，也不应该靠这种实验来推断是否需要增强针。我们可以回过头看一下循证医学里的证据等级层次，随机双盲的三期临床试验肯定要高于体外实验的，血清中和实验说到底就是一个体外实验，有其意义，但不能拿这个来推翻证据等级更高的三期临床试验结果。另外，以色列现在说辉瑞的有效性下降到39%了。仍然没有公布具体研究方法与详细数据。这是根据6月20日到7月17日的研究，比之前的64%大概晚一个月。即便是抗体下降，一般除了最初两个月大幅下降，后面降幅会缓和，所以以色列这种每隔一个月有效性减少个30%，我是觉得很难理解。特别在欧美，Delta导致的感染仍然大量集中在没接种疫苗的人里，所以以色列这个结论我们还是等等看比较好。这些仓促的结论很容易引起焦虑，比较遗憾，但现在确实也是特殊时期，这种情况很难避免。以前很多科学研究都是在科研界的小圈子里讨论、分析，很多时候有更多的时间来从科学上探讨，如果结论下得比较大，会有一个同行辩论、纠正的机会。不会像现在，一下子就引起全社会关注的。现在这样有利有弊，有利的地方是确实可以提供更及时的讯息，弊就是刚才提到的，有时候也是把一些仓促有问题的标题当科学结论传播了，造成不必要的误解与焦虑。在整个过程中，有一点比较遗憾的，美国这里的疫苗真实有效性、安全性的原创性研究太少了。像对Delta的有效性，我们引用的是英国的，对beta，引用卡塔尔的，美国这里做来做去，都是血清中和实验居多。这种情况是因为美国医疗系统的问题造成的。别人有很好很详细的电子医疗档案，及时做分析。美国这方面是有问题的。打了三亿多剂的mRNA疫苗，CDC拿出来的疫苗有效性是4000多人做的试验，突破性感染数据现在都没有对上基因组测序。这些都落后的有点荒唐。强生现在美国接种了1200万剂，换到以色列或英国，别说1200万剂，接种100万都有有效性跟踪数据了。哪怕一两周前发表在NEJM上的智利真实世界研究，确认科兴有效性的，也就1200多万人的记录。另外安全性问题，美国的VAERS与VSD不能算没起作用。但现在明显的一个问题是，要其它国家地区先发现问题，这两个系统才能根据这些问题去看看美国有没有同样的问题。血栓是欧洲先在阿斯利康里面发现的，心肌炎是以色列先找到的。本来以为GBS终于是VAERS先找到，但现在从时间看可能也是欧洲那边阿斯利康出了后才找到的。CDC在安全性上很透明很好，但在研究上还是落后了。由于美国医疗系统这样的缺陷，所以现在虽然美国打了很多疫苗，我们大部分科学证据仍然依赖英国、以色列等国的数据。但即便如此，这并不影响疫苗的证据充足度。现在面对反复的疫情，最好的措施仍然是及时接种疫苗。--- Support this podcast: https://anchor.fm/fg1d7g84g68/support2021-07-2533 min
