JAMA Neurology, Published online August 4, 2025論文の要約本研究は、米国テネシー州のMedicaid加入小児(5〜17歳)約69万人を対象に、インフルエンザ罹患時およびオセルタミビル治療の有無によって、神経精神系の重篤な有害事象(入院を要するもの)の発生リスクに差があるかを検討した前向きコホート研究である。方法:インフルエンザ流行シーズン(2016–2017年〜2019–2020年)における約1,960万パーソンウィークのデータを用い、曝露(治療なし・オセルタミビル治療・治療後期間・予防的使用・未曝露)ごとに分類し、Poisson回帰で補正後リスクを算出。主要結果:インフルエンザ罹患期間において、オセルタミビル治療ありの群では、治療なし群に比べて重篤な神経精神イベントのリスクが約50%低下していた(aIRR 0.53, 95% CI 0.33–0.88)。治療終了後の期間でも同様にリスク低下(aIRR 0.42, 95% CI 0.24–0.74)。特に神経学的イベント(例:痙攣、意識障害など)で有意な低下が認められた(aIRR 0.45)が、精神症状(自傷、気分障害など)では有意差はなかった。追加解析と感度分析:定義変更や共変量除外、交絡の影響を考慮した分析においても結果は一貫しており、観察されたリスク低下が偶然やバイアスによるものではないことが示唆された。臨床的意義:オセルタミビルは神経精神症状のリスクを高めるとの懸念が過去に報告されてきたが、本研究ではむしろインフルエンザ自体がリスク因子であり、オセルタミビルはその合併症を減少させる可能性があることが明らかになった。
