くつ王レディオ日本承認抗菌薬に対するASCスコア設定とDASC活用の全国サーベイランス統合に向けた検討　Journal of Infection and ChemotherapyAssignments of antibiotic spectrum coverage scores of antibiotic agents approved in Japan: Utilization of the days of antibiotic spectrum coverageCitationJournal of Infection and Chemotherapy, 2025; 31: 102667論文の要約本研究は、抗菌薬使用評価指標であるDASC（Days of Antibiotic Spectrum Coverage）を日本の全国サーベイランス（J-SIPHE）に組み込むため、従来スコア設定がなかった日本承認薬に対し新たにASC（Antibiotic Spectrum Coverage）スコアを付与し、そのAWaRe分類との関連を解析した。背景DASCはDOT（Days of Therapy）にASCスコアを掛け合わせ、抗菌薬の使用量とスペクトラムの広さを同時に評価できる指標。これまでのASCスコアは主に米国承認薬を対象としており、日本で日常的に使用される薬剤の多くが未設定だった。J-SIPHEではDOTやDDD、AWaRe分類は導入されているが、スペクトラム幅を直接反映する指標は未統合。方法日本で承認され、過去研究でASCスコア未設定の抗菌薬を対象に、16微生物カテゴリーに対する活性を文献・添付文書・専門家合意に基づき評価。経口剤と注射剤で有効成分が同一の場合は同じスコアを付与。AWaRe分類との相関をSpearmanの順位相関係数で解析。結果ASCスコア範囲は3〜16（中央値7）。AWaRe分類との相関は弱く（ρ=0.234）、両者は異なる視点から抗菌薬使用を評価することが示唆された。例：アンピシリン/スルバクタム（Access）は広域活性でASC=8、セフィデロコル（Reserve）はグラム陰性菌特異的でASC=5。意義新たなスコア設定により、日本で使用される全承認抗菌薬のDASC算出が可能に。DOTとの併用により、広域薬の削減やde-escalation評価をより精緻に行える可能性。AWaReとDASCは補完的関係にあり、両方を併用することで抗菌薬適正使用（ASP）の評価精度向上が期待される。

Corona hat unser Vereinsleben stark verändert und es gibt schon wieder ein neues, aktuelles Thema, das uns aufgrund der Pandemie in Atem hält. Wir werden am 12. April 2021 ein Testzentrum auf den Zietenterrassen eröffnen.
