Bivalent Prefusion F Vaccine in Pregnancy to Prevent RSV Illness in InfantsCitationNew England Journal of Medicine, 2023; 388:1451–1464論文の要約本多国間第3相二重盲検無作為化試験(MATISSE試験)は、妊娠24〜36週の妊婦に二価RSVプレフュージョンFタンパク質ワクチン(RSVpreF)を単回接種し、生後180日までの乳児におけるRSV関連下気道感染症の予防効果と安全性を評価した。対象と方法18か国、7,392名の妊婦を1:1でワクチン群(3,682例)またはプラセボ群(3,676例)に割り付け。主要評価項目は「生後90・120・150・180日までのRSV関連重症下気道感染(医療受診あり)」と「RSV関連下気道感染(医療受診あり)」の発症率。結果重症RSV-LRTD:90日以内は6例(ワクチン群) vs 33例(プラセボ群)、有効率81.8%(99.5%CI 40.6–96.3)、180日以内は有効率69.4%(97.58%CI 44.3–84.1)で主要評価の成功基準を満たした。RSV-LRTD(全重症度):90日以内は有効率57.1%(99.5%CI 14.7–79.8)で基準未達、180日以内は有効率51.3%(97.58%CI 29.4–66.8)。RSV関連入院:90日以内は有効率67.7%、180日以内は56.8%。全原因の下気道感染やRSV以外の呼吸器感染には有意差なし。安全性妊婦の局所反応(接種部位痛 41% vs 10%)や一部の全身症状(筋肉痛・頭痛)はワクチン群でやや多かったが、多くは軽度〜中等度で一過性。重大な有害事象や乳児の安全性に懸念は認められなかった。臨床的意義妊娠後期のRSVpreFワクチン接種は、生後6か月間の乳児における重症RSV下気道感染および入院を有意に減少させ、安全性も良好であった。出生直後からの保護が可能であり、RSV流行期の乳児予防戦略において有望な選択肢となる。
