DTBR#3: Ready for Pediatric ECMO + ECPR
Dr. Catherine Allan, Director of the Cardiac Care Unit and Inpatient Cardiology at the Cleveland Clinic joins us to talk about readiness for teams to perform pediatric ECMO, a high-risk, high-complexity therapy that staff might only see a third as often as they see patients on ventilators. ECMO can also be called for during CPR, which greatly increases the time pressure and complexity of the procedure. During ECPR, there is not only the ICU resuscitation microteam but also the surgical team and the perfusion team, leading to potentially having up to 20 people working in the room when running an ECPR...

Impact of ECPR initiation time and age on survival in out-of-hospital cardiac arrest patients: a nationwide observational study
Are we giving our older patients with out-of-hospital cardiac arrest (OHCA) a fair shot? ⏱️ Current guidelines say an ECPR initiation time of up to 60 minutes is acceptable, but is that really the case for everyone? This is a question clinicians grapple with every day at the bedside. A new nationwide observational study from South Korea tackles this head-on, analyzing data from 483 adult patients who received ECPR for non-traumatic OHCA. The study found that while both age and time to ECPR independently predict survival, the combination of the two is critical. The key takeaway? The "golden hour" for ECPR...

This episode of PEM Currents discusses ECPR (Extracorporeal Cardiopulmonary Resuscitation), an advanced procedure used in cases of cardiac arrest when traditional CPR fails. ECPR involves using ECMO (Extracorporeal Membrane Oxygenation) to take over heart and lung functions, offering a last-resort option that is becoming more common in large pediatric hospitals. While ECPR shows promise in improving survival rates, particularly in pediatric patients with conditions like congenital heart disease, it is resource-intensive and carries significant risks. Establishing an ECPR program requires robust infrastructure, multidisciplinary teamwork, and extensive training. The episode highlights the importance of understanding eCPR as a critical therapy...

Speaker – Andrew Geddes, European University Institute What factors shape European and EU responses to potential Afghan displacement and refugee movement towards the EU? In this presentation, it will be argued that these responses form part of a repertoire of migration governance that is grounded in how European and EU governance systems as organisational processes make sense of international migration. Rather than international migration in its various forms being something that simply happens to governance systems – an external challenge – to which they respond, governance systems are shown to be powerfully constitutive of international migration through their actions and inactions and throug...

Second, it analyses how corruption facilitates (irregular) migration, including human trafficking and smuggling. Third, several studies investigate how migration and the...
