糖尿病性足感染症とMRSA:鼻腔スクリーニングで「ない」を知る!抗菌薬適正使用の新常識 Antimicrobial Agents and Chemotherapy
Utility of Methicillin-Resistant Staphylococcus aureus Nares Screening for Patients with a Diabetic Foot InfectionCitationAntimicrobial Agents and Chemotherapy, 2020; 64(4): e02213-19概要米国退役軍人医療システムにおいて、2007〜2018年の糖尿病足感染症患者5,403名(8,163エピソード)を対象に、MRSA鼻腔スクリーニングの陰性的中率(NPV)と、感染部位からの培養結果との関連を後ろ向きに検討した。対象は全例で入院時または転棟時に鼻腔スワブが採取され、7日以内に足または足趾からの臨床培養が行われた。結果MRSA検出率は7.5%、MSSAは24.8%で最も多く、次いでEnterococcus(14.7%)が多かった。MRSA鼻腔スクリーニングの全体NPVは89.6%(深部培養89.2%、表層培養90.3%、足由来培養89.7%、足趾由来培養89.4%)。地域別や2007〜2012年と2013〜2018年での差はほぼなし(いずれも約90%前後)。陽性的中率(PPV)は43〜53%と低く、鼻腔保有が必ずしも局所感染を意味しない。考察鼻腔MRSA非保有であれば、その後の足部培養でMRSAが検出される可能性は約10%以下であり、特に重症でない患者では経験的な抗MRSA薬投与の中止や回避に利用できる可能性がある。抗MRSA薬の過剰使用による腎毒性や耐性菌リスク低減にもつながる。結論糖尿病足感染症において、直近の鼻腔MRSAスクリーニング陰性結果は、MRSA感染の除外に高いNPVを示し、抗菌薬適正使用(特にバンコマイシン回避)のための有用な指標となる。
