Methicillin-resistant and susceptible Staphylococcus aureus: tolerance, immune evasion and treatmentCitationNat Rev Microbiol. 2025; doi:10.1038/s41579-025-01226-2概要本総説は、メチシリン耐性黄色ブドウ球菌(MRSA)およびメチシリン感受性黄色ブドウ球菌(MSSA)の感染に関する最新の理解をまとめ、特に薬剤耐性に加えて「トレランス」「免疫回避」といった病態形成因子に焦点を当て、治療戦略の展望を論じている。1. 耐性とトレランス従来、ブドウ球菌感染症の難治化は主に薬剤耐性に帰せられてきた。しかし近年、殺菌的抗菌薬に曝露しても死滅せず休眠状態で生き残る「トレランス」や「パーシスター細胞」の役割が注目されている。これらは遺伝的耐性とは異なり、臨床的再燃や慢性感染の背景因子となる。2. 免疫回避機構S. aureus は多様な毒素や表面タンパクを介して宿主免疫から逃避する。代表的な機序として、補体や好中球による食作用阻害、免疫修飾タンパク質(Protein A, SCINなど)による抗体応答の回避が挙げられる。また、バイオフィルム形成は抗菌薬浸透を妨げるとともに、局所免疫応答を抑制する。これらの戦略はMSSA、MRSAに共通してみられる。3. 臨床像と治療の課題黄色ブドウ球菌は皮膚軟部組織感染から菌血症・心内膜炎に至る幅広い感染症を引き起こす。MRSAは依然として世界的に重要な院内感染起因菌であり、MSSAも同等に高い罹患率と死亡率をもたらしている。治療はβ-ラクタム系(MSSA)やバンコマイシン・リネゾリド・ダプトマイシン(MRSA)が用いられるが、トレランスやバイオフィルム感染では治療失敗が多い。近年は併用療法(例:ダプトマイシン+β-ラクタム)が有効とする報告があり、殺菌活性の増強が試みられている。4. 新規治療戦略抗菌薬の最適化に加え、免疫療法や抗毒素抗体、ファージ療法、免疫賦活薬などの開発が進められている。また、耐性だけでなく「トレランス」を標的とする薬剤候補の探索も始まっており、慢性再発性感染の克服につながる可能性がある。結論S. aureus 感染の難治化は耐性菌出現のみならず、トレランスや免疫回避の複合要因により説明される。これらの理解を統合し、抗菌薬治療・免疫調節療法・新規アプローチを組み合わせることが、将来の治療成績改善に不可欠である。
