Adjunctive Daptomycin in the Treatment of Methicillin-susceptible Staphylococcus aureus Bacteremia: A Randomized, Controlled TrialCitationClinical Infectious Diseases, 2021; 72(9): e196–e203概要カナダ・モントリオールの2施設で、成人MSSA菌血症患者を対象に、標準治療(抗ブドウ球菌βラクタム:クロキサシリンまたはセファゾリン)に加え、5日間のダプトマイシン補助投与が有効かを検証した無作為化二重盲検プラセボ対照試験(DASH試験)。対象・方法登録患者104例(中央値67歳、34.6%女性)。主評価項目:菌血症持続期間(日数)。副次評価項目:90日死亡率、再発率、塞栓/転移性感染、腎毒性・肝毒性・横紋筋融解症発症率。結果全解析集団(ITT)での菌血症持続期間中央値はダプトマイシン群2.04日、プラセボ群1.65日で差はなし(P=0.40)。90日死亡率はダプトマイシン群18.9%、プラセボ群17.7%で有意差なし。再発率、塞栓・転移性感染発生率にも差はなかった。有害事象発生率は両群で同等で、重篤な薬剤関連有害事象はなし。結論MSSA菌血症における標準βラクタム療法へのダプトマイシン併用は、菌血症持続期間や死亡率、再発率を改善せず、 routine での使用は推奨されない。
