關於Omicron變種病毒的消息,資訊肯定會變化很快,部落格文章我會隨時更新。 https://linshibi.com/?p=39815 1.Omicron變異株為何讓科學家特別擔心? 此變異株的棘狀蛋白上有32個突變,集先前各種變異株的突變特徵於一身,包括與病毒進入細胞,免疫逃脫或是感染力增加等等有關的突變。且初步在南非已經證實可以很快的取代原本的Delta變成主流病毒株,證實了他的能耐。 2.Omicron傳播力比Delta更強嗎?為何你說這個魔王可能是玩真的? 之前的幾個假魔王(比方Delta plus,Mu,C1.2.)都沒有辦法對Delta造成太大的威脅。但Omicron新變異株最早在南非及波札那發現,近2週相關病例數在南非東北的豪登省快速攀升,取代原本流行的Delta變異株,預估可能成為下一波南非主流病毒株,WHO判斷已經符合了高關注變異株(Variants of Concern, VOC)其中一個定義:新冠的流行病學上產生了不利的改變,因此決定將B.1.1.529界定為高關注變異株,命名為Omicron。這是第五株VOC,加入了Alpha、Beta、Gamma和Delta變異株的行列。之前的假魔王,沒有一個被界定為VOC。而且Omicron是短短14天內就被界定為VOC。 3.Omicron會不會更容易重症? 南非初步報告說以輕症為多,但你要知道這個病毒從11月12日才在南非第一次偵測,且一開始以年輕人為多,也有些人是二次感染。因此現在觀測到輕症居多是正常的。可能需數週時間累積更多臨床資訊,分析在老人家或有重症風險因子,還有沒有打疫苗者是否會有更高的重症風險。之前不管是Alpha或是Delta,我們都是大概兩個月以後才有比較確認的研究。 我真心希望雖然Omicron變得比較有傳染力,但疾病的嚴重度若是會變輕的話,這反而是這個病毒往輕症化,流感化的方向走呀。 4.新冠疫苗對Omicron還有用嗎?單株抗體,口服藥的效力呢? 現有新冠疫苗幾乎都是針對病毒的棘狀蛋白產生抗體,而Omicron變異株的棘狀蛋白上就有32個突變,很令人擔心疫苗能夠產生的中和抗體是否會更低。各疫苗公司已經開啟實驗室研究,初步的結果應該兩週左右會出來。南非NICD是預估,也許會有一些免疫逃逸,但疫苗應該對於重症和死亡還是可以提供高的防護力的。 就算很不幸現有疫苗完全失效了,mRNA的技術平台也可以很快地做出新疫苗。輝瑞/BNT和莫德納疫苗六週的時間就可以改變mRNA序列做出新疫苗,輝瑞聲稱可以在100天內開始出貨。 Omicron變異株在單株抗體所作用的受體結合區域(Receptor-binding domain; RBD)上有很多個突變。因此單株抗體的確有可能會效用降低,這需要後續研究。至於口服藥,因為其作用機轉和疫苗完全不同,不是作用在棘狀蛋白上,因此很可能是不太受影響的。但這當然也需要後續研究證實。 5.各國如何因應?國際交流又會就此停止嗎? 目前仍以入境限制為主。英國開第一槍禁航。美國11月29日起限制南非等8國旅客入境,並呼籲民眾盡快施打疫苗追加劑。歐盟建議對非洲南部7國人士實施旅行限制。日本強化對非洲南部9國入境管制,相關人士須在指定場所隔離10天。 台灣指揮中心將非洲南部6國:南非、波札那、納米比亞、賴索托、史瓦帝尼、辛巴威列重點高風險國家,11月29日0時起自上述國家入境或轉機的旅客,須入住集中檢疫所並配合專案採檢。陳時中指揮官表示,現階段國際驗出新變種病毒確診個案人數少,且台灣沒有直飛航班,直接威脅相對小,還是要以完整阻絕為先,會持續關注國際狀況。他也呼籲國人盡速接種疫苗,目前75歲以上長者仍有27%未打疫苗。 南非衛生部批評各國的管制措施太過嚴苛、不科學且違反WHO建議。這很似曾相識,像去年12月英國宣布alpha之後全世界幾乎都對英國禁航。但在這株病毒的擴散範圍,還有詳細特性被釐清之前,各國採取比較謹慎的措施,我認為是合理的。至於會持續多久,就要看之後對此病毒的了解,還有此病毒的擴散狀況了。 6.現有PCR可以偵測Omicron變種株嗎? 目前的PCR是可以檢測出此變異株的,沒有漏掉的問題。南非的幾個實驗室反應主要會檢驗的三段基因中,其中的S基因會檢測不到Omicron(稱之為 S gene dropout或 S gene target failure),這可以在完整定序之前較快速的辨識出這大概是Omicron。 7.台灣的防疫需要做什麼改變? 先不用太擔心,維持目前阻絕疫情於境外的策略。指揮中心會根據最新資訊隨時滾動式調整的。而民眾,做的事情都一樣。出外戴口罩,勤洗手,盡可能保持社交距離主,注重室內通風。能打疫苗的,連第一針都還沒打的,第二針還沒打的,快點去打。我們明年準備要打加強針了。 林氏璧醫師的電子名片 https://lit.link/linshibi 小額贊助支持本節目: https://open.firstory.me/join/linshibi

The National Health Department has confirmed the prevalence of measles cases in five provinces - Gauteng, Mpumalanga, KwaZulu-Natal, Northern Cape and Western Cape. Health ministry spokesperson Joe Maila says about 49 laboratory confirmed measles cases have been noted since the beginning of this year. He says majority cases were reported in the Northern Cape with 18 measles detected up-to-date.
