岡大徳のポッドキャスト【中医協解説】令和8年度改定に向けた4つの重要論点｜訪問看護・特定疾患療養管理料・身体的拘束・廃用症候群令和7年12月19日に開催された中央社会保険医療協議会 総会（第637回）では、これまでの審議で委員から出された指摘事項に対する回答として、追加データが提示されました。本メールマガジンでは、訪問看護、特定疾患療養管理料、身体的拘束、廃用症候群の4つの論点について解説します。今回の議論では、訪問看護における同一建物利用者への評価見直し、特定疾患療養管理料の算定実態、療養病棟等における身体的拘束の最小化、障害者施設等入院基本料における廃用症候群患者の評価について、NDBデータや実態調査に基づく詳細な分析結果が示されました。これらのデータを踏まえ、令和8年度診療報酬改定に向けた具体的な検討が進められています。訪問看護に係る指摘事項への回答中央社会保険医療協議会では、高齢者住まい等に居住する利用者への訪問看護について、同一建物・単一建物利用者の人数や訪問回数に応じた評価のあり方が論点となっています。この論点に関して、追加データが提示され、具体的な検討の方向性が示されました。10月1日の総会では、委員から3つの指摘がありました。第一に、評価の見直しについては、コストだけでなく個々の患者の医療の必要度や提供サービスの内容も加味したきめ細かい評価が必要という意見です。第二に、頻回訪問の利用者の9割は末期の悪性腫瘍や別表第７該当者であることを踏まえるべきという指摘です。第三に、高齢者住宅等への頻回訪問については、一連の訪問看護について包括的な評価の検討が必要という意見が出されました。これらの指摘を受け、今回の総会では追加データが示されました。訪問看護基本療養費Ⅱを算定する利用者は、別表第７該当の有無や疾病によらず、1月あたりの訪問日数、訪問回数、難病等複数回訪問加算の算定日数が多い傾向にあります。別表第７に該当する利用者の1月あたり医療費の平均は172,003円で、それ以外の利用者（79,536円）のほぼ2倍となっています。さらに、訪問看護ステーションごとに別表第７該当利用者の割合が8割以上のステーションでは、利用者1人あたりの1月あたり医療費が277,188円と、2割未満のステーション（87,010円）より高額です。訪問看護管理療養費の算定種別による違いも明らかになりました。月の2日目以降の訪問看護管理療養費をみると、訪問看護管理療養費１のみの利用者の医療費中央値は90,849円であるのに対し、訪問看護管理療養費２のみの利用者は192,162円と高い傾向です。ただし、訪問看護管理療養費１と２の利用者における別表第７該当者の割合には大きな差異はありませんでした。今回の論点として、高齢者住まい等に併設・隣接する訪問看護ステーション等による同一建物・単一建物利用者への訪問看護について、頻回または画一的に加算等が算定される場合の評価水準や、複数回の訪問の時間を合算する考え方等を含め、具体的な評価のあり方が検討されています。特定疾患療養管理料に係る指摘事項への回答特定疾患療養管理料については、主傷病名の分析に加えて副傷病名や処方状況を含めた調査・分析が求められていました。今回の総会では、NDBデータに基づく詳細な分析結果が提示されました。10月17日の総会での指摘は2点でした。一つは、主傷病名の分析に加え、副傷病名や処方状況も含めたさらなる調査・分析が必要であり、適切な運用が行われていない場合には制度の是正を図るべきという意見です。もう一つは、特定疾患療養管理料についてはこれまでの議論の積み重ねを踏まえた制度であり、その意義を十分に理解すべきという意見でした。これらの指摘に対し、今回は主傷病名、全傷病名、処方状況に関するデータが示されました。主傷病名の上位は、気管支喘息（12.8%）、慢性胃炎（9.2%）、狭心症（6.0%）となっています。全傷病名（主傷病名と副傷病名を含む）でみると、高血圧症（49.7%）、慢性胃炎（27.5%）、アレルギー性鼻炎（26.2%）の順となっており、令和6年度改定で対象から除外された高血圧症や糖尿病（19.9%）が副傷病名として多く記載されている実態が明らかになりました。処方状況については、特定疾患療養管理料算定患者のうち、処方料・処方箋料のいずれも算定がない患者は3.8%でした。特定疾患処方管理加算の算定がある患者は65.8%となっています。また、主傷病名が「胃潰瘍」に関連する患者のうち、非ステロイド性抗炎症薬（NSAIDs）の内服薬を調剤されている患者が約6.5%存在することも示されました。NSAIDsは胃潰瘍のリスク因子であることから、療養管理の実態について検討が求められています。身体的拘束の最小化に向けた取組に係る指摘事項への回答身体的拘束の最小化については、回復期リハビリテーション病棟と療養病棟における実施状況の詳細分析が求められていました。今回の総会では、拘束の種類やデバイスの有無、認知症の有無による違いを含めた詳細データが提示されました。10月29日の総会では、2つの指摘がありました。回復期リハビリテーション病棟については、身体的拘束の中にはクリップセンサーや離床センサーといった見守り目的のものが含まれており、物理的に四肢を固定する拘束とは異なることから、拘束の種類を考慮すべきという意見です。療養病棟については、身体的拘束の実施状況に施設間で差があることについて、患者の状態、認知症の程度、使用しているデバイス等の詳細な分析が先決という指摘でした。追加データとして、常時の手指・四肢・体幹抑制とそれ以外の身体的拘束（クリップセンサー等を含む）を区分した分析結果が示されました。回復期リハビリテーション病棟では、身体的拘束全体では20%以上の患者に実施している病棟が25.0%あるのに対し、常時の手指・四肢・体幹抑制に限ると12.7%にとどまります。このことから、クリップセンサー等による行動把握が多く行われていると考えられます。ただし、1割以上の病棟では常時の拘束を20%以上の患者に実施していました。療養病棟では、デバイスの有無と認知症の有無による違いが明確になりました。デバイスあり・認知症ありの患者では、身体的拘束の実施率について医療機関間の差が大きいことがわかりました。デバイスも認知症もない患者では、常時の拘束実施がゼロの病棟が95.6%と多い一方、デバイスがなくても認知症がある場合は一定数の病棟で常時の拘束が実施されており、そのほとんどは認知症高齢者の日常生活自立度Ⅳ・Mの患者でした。デバイスの種類別では、胃ろうの場合は身体的拘束実施率0%の病棟が54.8%と比較的多いのに対し、経鼻胃管やIVH（中心静脈栄養）の場合は拘束実施率の高い病棟が多いという結果でした。この背景には、胃ろうより経鼻経管栄養や中心静脈栄養を希望される傾向が増えていることがあります。今回の論点として、身体的拘束の最小化に向けた評価に際して留意すべき点について検討が進められています。障害者施設等入院基本料における廃用症候群に係る指摘事項への回答障害者施設等入院基本料における廃用症候群患者については、病態による違いを踏まえた詳細な分析が求められていました。今回の総会では、身体障害者障害程度等級による看護提供頻度の違いを示すデータが提示されました。10月29日の総会では、障害者施設等入院基本料の10対1から15対1において廃用症候群の患者割合が多く療養病棟と類似している点について指摘がありました。委員からは、障害者施設等入院基本料の対象患者は身体障害者障害程度等級の1級・2級相当であり、脊髄損傷など障害が固定した状態にあり廃用症候群からの回復が困難な場合も多いとの意見が出されました。一方で、器質的障害のない廃用症候群はしばしば改善することもあり、同じ廃用症候群でも病態により異なるため、より詳細な分析が必要との指摘でした。追加データとして、廃用症候群を主傷病とする患者に対する看護提供頻度の分析結果が示されました。障害者施設等入院基本料を算定する患者のうち、身体障害者障害程度等級表の1・2級以外の患者では、療養病棟と看護提供頻度の分布が類似していました。これに対し、1・2級の患者においては、より頻回な看護提供を必要とする患者の割合が高いという結果でした。このデータは、同じ廃用症候群であっても、重度の身体障害を有する患者と器質的障害のない患者では、看護の必要性が異なることを示しています。今後の評価のあり方については、こうした病態の違いを踏まえた検討が進められる見込みです。まとめ中央社会保険医療協議会 総会（第637回）では、訪問看護、特定疾患療養管理料、身体的拘束の最小化、障害者施設等入院基本料における廃用症候群の4つの論点について、追加データに基づく議論が行われました。訪問看護については、同一建物利用者への頻回訪問の評価水準や複数回訪問の時間合算等の具体的な検討が進められます。特定疾患療養管理料については、主傷病名・副傷病名・処方状況の分析により
