Published online September 1, 2025. doi:10.1093/cid/ciaf429概要本研究は、過体重または肥満の成人において、COVID-19罹患後のPost–COVID-19 Condition(PCC、いわゆるLong COVID)発症リスクに対するメトホルミン投与の効果を検証した大規模後ろ向きコホート研究である。デザイン:ターゲットトライアルエミュレーションを用いた後ろ向きコホート研究。データソース:英国Clinical Practice Research Datalink Aurum(2020年3月~2023年7月)。対象:BMI ≥25 kg/m²の成人624,308人。直近1年以内にメトホルミン使用歴や禁忌がある者は除外。介入:COVID-19診断後90日以内にメトホルミンを新規開始した群(n=2,976)と、非使用群(n=1,866,637)を比較。アウトカム:診断後90〜365日にPCC診断コード、またはWHOが定義する25症状の少なくとも1つが新規に出現した場合をPCCと定義。主要解析:ITT解析でのPCC発症1年リスク差 −12.58%、HR 0.36(95%CI 0.32–0.41)。PP解析でもほぼ同様(RD −12.74%、HR 0.36)。サブグループ解析(表3, p.8):年齢:45歳未満 HR 0.23、45歳以上 HR 0.42。性別:男性 HR 0.36、女性 HR 0.38。糖尿病の有無:非糖尿病 HR 0.33、糖尿病 HR 0.39。BMI:過体重 HR 0.34、肥満 HR 0.39。SARS-CoV-2株:デルタ期 HR 0.33、オミクロン期 HR 0.32。→ 全ての層で一貫したPCCリスク低下が確認された。感度解析(表4, p.8):癌発症をネガティブコントロールとした解析では関連なし(HR 1.13)、結果の頑健性を支持。図2(p.7):累積発症曲線において、メトホルミン群は非使用群より明らかにPCC発症率が低い傾向を示した。メトホルミン開始は、過体重・肥満成人においてCOVID-19後のPCCリスクを有意に低減させた。抗炎症作用やSARS-CoV-2に対する抗ウイルス作用が機序の候補とされるが、因果関係を確定するにはさらなるRCTが必要である。一方で、正常体重者への一般化はできず、投与量や製剤の影響も未評価である。COVID-19後に過体重・肥満の成人がメトホルミンを早期に開始することで、PCC発症を抑制できる可能性がある。低コストで安全性の高い薬剤として、Long COVID対策に寄与する可能性が示唆された。方法結果考察結論
